A Center for
Health Education, Referral and Advocacy
Initiated
by Faith in the City, the Wellness Connection of Minnesota is a
collaborative project by
community service agencies,
health care providers, and faith
communities in Minneapolis. Our
vision is to increase the
quality of life within the
Somali refugee and high-risk
populations in the Phillips and
surrounding neighborhoods. Our
goal is to create an
increased quality of health and
wellness for individuals and
their families.
Q. What is the
Wellness Connection?
The Wellness Connection will
provide a link to the Minnesota
health care system. People who
come to the center will learn
how our health care system
works, how to access it, and
how to determine and locate
appropriate resources.
Services include health
education, assistance getting to
appointments, completing
paperwork, referrals, and other
services based on need.
Q. How much does it cost?
The Wellness Connection is
free to all visitors.
Q. What is the mission?
To provide knowledge and
to teach skills so people can
independently access appropriate health care.
Q. Who is the center
for?
The center is for any refugee,
immigrant, or high-risk families
or individuals in the
Minneapolis-St. Paul metro area.
Q. Who assists the clients?
Four Somali bi-lingual health
care workers, volunteers, and a
program coordinator will
manage the center’s daily
activities.
